HELP FOR ADULTS & TEENS
CHD’s outpatient programs offer compassionate, respectful and effective pathways leading to the JOY of sobriety and LIVING drug free. Our Sonoma County services are available in Santa Rosa, Guerneville, and Cloverdale. We offer evaluation, assessment and individual treatment plans; individual and group counseling sessions with groups for men, women and teens; referrals to community services such as housing, employment, health coverage, and food and legal assistance; and transition planning to prevent relapse. Services are offered in English and Spanish and are bicultural in approach. Our clients gain a clinical understanding of their addiction and build new life skills to support long-term sobriety.
Personal commitment and courage are key elements of success in our programs, which include Outpatient Treatment, DUI Treatment Court, and Teen Treatment.
